Silybum marianum

Description
White-veined, marbled leaves with stout spines and purple flower heads mark out this thistle-like plant of the daisy family (Asteraceae) — legend has it the white veins stem from a drop of the Virgin Mary's milk. The fruits, ripening in late summer, are used; they contain the flavonolignan complex silymarin (1.5-3%) as well as isolated silibinin. Commission E recognises tinctures made from milk thistle fruit for digestive complaints and as supportive treatment for toxic liver damage; clinical trials have examined silymarin preparations in various liver conditions. IMPORTANT: silymarin supports the liver but does not replace medical treatment — signs such as jaundice, dark urine or abdominal pain call for immediate medical attention. Milk thistle preparations are not recommended for children and adolescents under 18, and self-medication should not exceed four weeks.

STANDARDISED DRY EXTRACT — EMA well-established use: Standardised preparations of milk thistle fruit (Silybi mariani fructus), dry extracts as capsules or coated tablets, DER 36–44:1, standardised to 30–65 % silymarin (as silibinin). Examples: Legalon 70/140 mg, Silymarin Stada. EMA dosage: 70–420 mg silymarin daily in 2–3 divided doses, with meals. Well-established indication: supportive treatment of toxic liver damage and chronic inflammatory liver diseases including cirrhosis. Onset 2–4 weeks; consult doctor for acute complaints. Do not self-treat jaundice or ascites — these need immediate medical assessment.

Per EMA monograph (traditional use): supportive treatment of dyspeptic complaints (bloating, flatulence, mild digestive discomfort). Prefer standardised preparations over raw powder for bioavailability. No improvement after 4 weeks: consult doctor.

Folk tea from freshly crushed fruits for general liver support — silymarin poorly water-soluble, but digestive companions dissolve effectively.
Preparation & dosage
FOLK INFUSION: 3–5 g freshly crushed milk thistle fruits (rough crush in mortar — do NOT grind fine) in 200 ml boiling water, covered, steep 10–15 min, strain. 3× daily before meals. NOTE: Silymarin is very poorly water-soluble (~0.04 mg/ml). Tea delivers only a fraction of the effective dose of standardised extracts — used in folk medicine for general liver support but not a substitute for clinical extracts. Bitter compounds and mucilage dissolve in the infusion and support digestion.

Clinical use of i.v. silibinin hemisuccinate (Legalon SIL) in acute death cap mushroom poisoning — highest clinical evidence for liver-protective effect in amatoxin hepatotoxicity.

INTRAVENOUS USE IN HOSPITAL ONLY — Silibinin hemisuccinate (Legalon SIL): For acute amatoxin poisoning (death cap mushroom, Amanita phalloides), the isolated compound silibinin hemisuccinate is given as IV infusion. Dose: 20 mg/kg/day as continuous infusion, started as early as possible. This is NOT for self-treatment — suspected mushroom poisoning: call poison control immediately and go to A&E. Mechanism: inhibition of amatoxin hepatocyte uptake via OATP1B1/1B3. Also described for hepatotoxic drug injury management in clinical settings.

Young leaves (despiked), peeled stems and immature flower heads are edible — traditional vegetable in Mediterranean regions; nutritionally valuable for proteins, fat and flavonoids.
Preparation & dosage
EDIBLE PLANT — leaves and stems: Young rosette leaves (pre-flowering, March–April), remove spines (cut with scissors), eat raw as salad or briefly blanched (2–3 min) as vegetable. Taste mild, slightly bitter. Peel stems before flowering (like rhubarb) and cook as vegetable or candy. Immature flower heads prepared like artichokes. Traditional food plant in Mediterranean countries.
Positively assessed by Kommission E for liver toxicity, chronic hepatitis and cirrhosis (200–400 mg silymarin/day) — one of the best-documented herbal liver indications in Germany.

KOMMISSION E RECOMMENDATION — daily dose 200–400 mg silymarin (as silibinin) in standardised preparations. Positive Kommission E vote (1986, revised 1992) for toxic liver damage and supportive treatment of chronic hepatitis and cirrhosis. Single dose 140 mg silymarin 2–3× daily as capsule. Tincture (DAB, 1:5 in 45–60 % ethanol) matches fruit dosing but contains less bioavailable silymarin than dry extract — prefer capsules. Severe liver disease: medical treatment mandatory.

Folk leaf tea for digestive complaints and bile flow stimulation — low silymarin content; primary action through bitter compounds.

LEAF TEA (folk): 2–3 tsp dried, coarsely cut milk thistle leaves (remove spines before drying) in 250 ml boiling water, covered, steep 8–10 min, strain finely. 1–2× daily between meals. Bitter and tannin compounds stimulate bile flow and digestive secretions. Silymarin content in leaves is very low — leaf preparations support digestion generally, not liver therapy. PFAF documents folk use of leaves for liver and gallbladder complaints in Mediterranean traditions.

Hulled milk thistle seeds as nutrient-dense raw food ingredient: mild nutty flavour, protein, fatty acids and silymarin — traditional Mediterranean use and modern superfood.
Preparation & dosage
SEEDS in the kitchen: Ripe, hulled milk thistle seeds have a mild, nutty flavour. Raw as addition to muesli, salads or smoothies (1–2 tsp daily). Lightly toasted as topping on soups and vegetables. Ground to fine powder and stirred into yoghurt or quark (whole food: ~25 % protein, 20–30 % fat, silymarin). Valued ingredient in Mediterranean and health-food cuisine. Consume ground seeds promptly — oil oxidises quickly after grinding.
IMPORTANT: Silymarin extracts are very well tolerated, but liver disease requires medical care. Silymarin SUPPORTS — it does not cure. Jaundice, abdominal pain, dark urine, pale stools or ascites: see a doctor immediately — possible signs of serious liver disease. Milk thistle tea is NOT an emergency antidote for poisoning: for suspected death cap poisoning call poison control immediately and go to A&E — i.v. silibinin is a hospital procedure. Rare allergic reactions (cross-reactivity with Asteraceae possible). Occasional mild GI symptoms at start. Limit self-medication to 4 weeks (up to 8 weeks under medical supervision).

Drug interactions
CYP3A4-Substrate (Ciclosporin, Tacrolimus, bestimmte Statine, Indinavir, HIV-Proteaseinhibitoren)
High-dose silymarin extracts may inhibit CYP3A4 in intestinal mucosa and liver, increasing bioavailability of CYP3A4-dependent drugs. Particular caution with narrow therapeutic index immunosuppressants (ciclosporin, tacrolimus) and antiretrovirals. Low clinical risk at normal doses; inform doctor when on multiple medications.
P-Glykoprotein-Substrate (Digoxin, bestimmte Zytostatika, Talinolol)
In vitro data show P-glycoprotein inhibition by silymarin components — possible increased intestinal absorption of P-gp substrates. Clinical relevance at normal doses low; plasma level monitoring recommended with sensitive substrates (e.g. digoxin) at higher doses.
Antidiabetika (Metformin, Sulfonylharnstoffe, Insulin)
Clinical studies show silymarin may mildly reduce fasting glucose and HbA1c in type 2 diabetes (insulin-sensitising, gluconeogenesis inhibition). Additive blood glucose lowering risk with antidiabetics — monitor blood glucose regularly, dose adjustment may be needed.
Hepatotoxische Arzneimittel (Methotrexat, Amiodaron, Valproat, Isoniazid)
Silymarin may partially attenuate liver cell damage induced by hepatotoxic drugs (antioxidant, membrane-stabilising). This PROTECTIVE interaction does not increase toxicity — but may mask drug toxicity assessment (when transaminases are used as toxicity marker). Combine only under medical supervision.
Contraindications
- Hypersensitivity to milk thistle (Silybum marianum) or other Asteraceae/Compositae — cross-allergy with ragweed, chamomile, mugwort, marigold possible.
- Children and adolescents under 18.
- Obstructive biliary disease (biliary colic, obstructing gallstones) — bile flow stimulation may trigger colic.
- Hormone-sensitive conditions (breast cancer, uterine cancer, endometriosis, uterine fibroids) — silymarin may exhibit weak oestrogenic activity; clinical significance uncertain, caution advised.
Harvested part: ripe fruits
- Time of day:
- Dry sunny morning (9–12), once dew has fully evaporated. Ripe fruits are recognised by opening white pappus hairs (feathery crown) and seeds falling out on light touch — this marks the optimal harvest window, which lasts only a few days.
- Drying:
- CAUTION: Milk thistle is heavily spined — leather gloves, long sleeves and eye protection are essential. Cut ripe flower heads close to the stem, immediately place in linen or paper bags (pappus hairs scatter in wind and can irritate mucous membranes). Ripen heads 1–2 weeks upside down in paper bags in a dry, well-ventilated location. Then shake or tap heads over a large cloth to release seeds. Remove pappus hairs by sieving (fine sieve or cloth). Dry fruits in a single layer at max 40 °C in a dehydrator or air-dry — residual moisture below 10 %. Store whole, uncrushed fruits in airtight dark jars; crush only just before use (silymarin oxidises rapidly after grinding).
- Storage:
- 24 months — Whole (uncrushed) fruits dark, dry and airtight — shelf life up to 2 years. After crushing/grinding, use promptly (max. 2–4 weeks in sealed jar). Store standardised preparations per label instructions.
- Time of day:
- Dry, cool morning (8–11) for leaves — harvest young rosette leaves in early spring before the flower stalk emerges (March–April). Older leaves become tough and intensely bitter. Gloves necessary for leaf spines.
- Drying:
- Harvest young leaves, immediately remove spines with kitchen scissors (in kitchen) or despin completely before drying. Spread leaves in a single layer on cloths or screens in shade, dry at max 35–40 °C until papery and rustling. Turn regularly to avoid damp patches. Crumble dried leaves coarsely and store in dark screw-top jars. Fresh leaves can also be used directly as vegetable (blanched) or salad (raw, despined) — no drying needed.
- Storage:
- 12 months — Dried leaves airtight, dark, cool — use within 1 year. Discard if yellowing or musty smell develops.

Lead compound complex of milk thistle fruit. Silibinin (silybin, sum of A+B) constitutes 50–70 % of silymarin and is the pharmacologically most active component. Mechanism: inhibits receptor-mediated hepatocyte toxin uptake (including amatoxins) via OATP1B1/1B3 transporters; membrane stabilisation via phospholipid bilayer interaction; stimulation of RNA polymerase I (increased hepatocyte protein synthesis); radical-scavenging antioxidant. Very poorly water-soluble — tea barely effective, extracts required.

As semi-synthetic derivative (silibinin hemisuccinate, Legalon SIL) approved for intravenous treatment of acute amatoxin poisoning.
