Ginkgo family (Ginkgoaceae)
Description
Fan-shaped leaves, often faintly notched at the tip, are the botanical trademark of ginkgo, a living fossil whose form has changed little in over 200 million years and which is now planted as a street tree worldwide. The leaves, gathered in autumn, contain flavonol glycosides, ginkgolides and bilobalide. Standardised extracts are regarded by the EMA as well established for supporting concentration, memory and circulation in older age, and traditional use describes similar applications for tinnitus. Ginkgo extracts can inhibit blood clotting, so a doctor must be informed before surgery or when taking anticoagulants such as warfarin or aspirin. The fleshy seed coat of ripe fruit contains allergenic ginkgolic acids and should only be handled with gloves, while the seeds contain the neurotoxic compound ginkgotoxin and must not be eaten by children, during pregnancy, or by anyone with epilepsy.
CONTRAINDICATED during pregnancy
Ginkgo leaf extracts are contraindicated in pregnancy: anticoagulant effect increases bleeding risk at parturition; ginkgotoxin from seeds may be teratogenic. EMA and ESCOP recommend strict avoidance of therapeutic doses in pregnancy. Occasional consumption of small amounts of cooked nuts in Japanese/Chinese cuisine is sometimes tolerated, but not recommended during pregnancy.
CONTRAINDICATED for children
Standardised ginkgo leaf extracts not recommended for children and adolescents under 18 (EMA) — no safety data. Ginkgo seeds (nuts) strictly contraindicated in children: ginkgotoxin causes seizures particularly easily in children (lower seizure threshold, higher relative intake per kg body weight). Poisoning cases (some fatal) from mass consumption of ginkgo nuts in Japanese and Chinese children have been reported.

Standardised dry extract (EGb 761) for improving cognitive function and quality of life in mild dementia — the only ginkgo preparation with EMA well-established use status.
Preparation & dosage
STANDARDISED DRY EXTRACT (EMA well-established use): Standardised dry leaf extracts, e.g. EGb 761 (Tebonin) or LI 1370, DER 35–67:1, acetone 60 % V/V, standardised to 22–27 % flavonol glycosides and 5–7 % terpene lactones. Daily dose 120–240 mg, in 2–3 single doses with meals. EMA indication: improvement of cognitive function and quality of life in elderly patients with mild dementia. Onset after 4–6 weeks of continuous use. Do not self-medicate confirmed dementia without diagnosis — ginkgo does not replace treatment of neurodegenerative disease. Inform physician before surgery and when taking anticoagulants (bleeding risk).

Preparation & dosage
STANDARDISED DRY EXTRACT — peripheral circulation (EMA traditional use): As above (EGb 761 / LI 1370), 120–160 mg daily in 2 single doses. Indication: symptomatic treatment of peripheral arterial occlusive disease (PAOD) Fontaine stage II (increasing pain-free walking distance). Adjunctive to exercise training. Medical diagnosis mandatory before self-treatment. Onset 6–8 weeks; EMA treatment duration up to 6 months observed.

STANDARDISED DRY EXTRACT — tinnitus / vertigo (EMA traditional use): 120–160 mg daily in 2 single doses (EGb 761). Indication: symptomatic treatment of age-associated tinnitus and vertigo of vascular origin. Best evidence for vascular-origin complaints; less clear for purely idiopathic tinnitus. Medical assessment of cause required before starting. Not a substitute for specific ENT therapy. Sudden hearing loss or stroke: immediate emergency call.

Preparation & dosage
INFUSION (traditional use): 2–3 g dried, coarsely cut ginkgo leaves in 200 ml boiling water, covered, steep 10 min, strain. 2× daily with meals. IMPORTANT: A simple leaf infusion contains flavonol glycosides and ginkgolides, but at much lower and more variable concentrations than standardised extracts — clinical efficacy comparable to EGb 761 is not established for leaf tea. Tea form is traditional use (EMA traditional) for supporting circulation and mental freshness during temporary exhaustion. Beyond 8 weeks without improvement, consult a doctor.

Edible seeds (ginkgo nuts) — cooked only, max. 5–10 nuts/day for adults; raw seeds and seed coat toxic/allergenic (ginkgotoxin, ginkgolic acids).
Preparation & dosage
GINKGO NUTS (seeds, cooked): Remove the fleshy outer seed coat (sarcotesta) — CAUTION: contains ginkgolic acids, causes severe contact dermatitis (similar to poison ivy), wear gloves. Crack or boil the hard inner stone (the actual 'nut') until soft (10–15 min). Traditionally used in Japanese and Chinese cuisine in dishes such as chawanmushi, congee, as garnish with sushi/sashimi, briefly pan-roasted, or grilled in the shell. Raw seeds contain 4'-O-methylpyridoxine (MPN, 'ginkgotoxin') — neurotoxic in larger amounts (seizures possible). MAXIMUM for adults: no more than 5–10 cooked nuts per day. Children, pregnant women, and people with epilepsy or vitamin B6 deficiency: NO ginkgo nuts.
Folk ginkgo tincture for mental exhaustion and poor concentration — non-standardised, lower evidence level than EGb 761.
Preparation & dosage
FOLK GINKGO TINCTURE (1:5 in 50–60 % ethanol): 1–2 ml (~20–40 drops) 3× daily in water or juice. Folk use for mental exhaustion, difficulty concentrating, and general circulatory weakness. Not equivalent to standardised extracts — no defined flavonol glycoside content. Do not combine with anticoagulants or antiplatelet agents without medical advice. Never use in children. Due to ethanol content, avoid in alcohol dependence, liver disease, or epilepsy.

TCM-inspired leaf infusion for respiratory complaints and cough — limited Western evidence; do not discontinue prescribed asthma medications without medical advice.

FOLK RESPIRATORY INFUSION: 1–2 g dried ginkgo leaves in 200 ml hot water, steep 8 min, strain. 2× daily. In traditional Chinese medicine (TCM), ginkgo leaves and seeds are used for asthma, chronic cough and whooping cough — terpene lactones (especially bilobalide) are said to have bronchodilatory and anti-inflammatory effects. Clinical evidence for this indication in Western phytotherapy is limited. Do not use as substitute for prescribed asthma medications.

IMPORTANT: Ginkgo leaf extracts (especially EGb 761) can inhibit platelet aggregation and increase bleeding risk — always inform physician before surgery, dental procedures, and when taking anticoagulants or antiplatelet agents (warfarin, phenprocoumon, aspirin, clopidogrel, heparin). Fresh ginkgo seed coat (sarcotesta) contains ginkgolic acids — potent allergen causing contact dermatitis (similar to poison ivy/Anacardiaceae). Wear gloves when handling fresh fruits/seeds. Ginkgo seeds (nuts) contain ginkgotoxin (4'-O-methylpyridoxine) — neurotoxic in large amounts, seizures possible in children and adults. Consume only cooked and in small amounts (max. 5–10 nuts/day for adults). Epilepsy, vitamin B6 deficiency, pregnancy, and children: strictly avoid seeds.

Drug interactions
Antikoagulanzien und Thrombozytenaggregationshemmer (Warfarin, Phenprocoumon, ASS, Clopidogrel, Heparin, NOAK)
Ginkgolide B inhibits platelet-activating factor (PAF) and additively suppresses platelet aggregation. Clinical case reports of elevated INR and spontaneous bleeds (subdural haematomas, retinal haemorrhages) when combined with warfarin or aspirin. Clinically relevant risk — avoid combination or use only under close medical supervision with regular coagulation monitoring.
Antiepileptika (Phenobarbital, Phenytoin, Carbamazepin, Valproat)
Ginkgotoxin (4'-O-methylpyridoxine) from seeds antagonises vitamin B6 (pyridoxal-5'-phosphate), reduces GABAergic inhibition and may lower seizure threshold. Concurrent antiepileptic therapy with seed consumption increases seizure risk. Ginkgo leaf extracts (no ginkgotoxin) interact enzymatically via CYP enzymes (CYP3A4 induction), potentially altering antiepileptic serum levels.
SSRI / SNRI (Fluoxetin, Sertralin, Paroxetin, Venlafaxin)
Several case reports of serotonergic overactivation and bleeding events (e.g. cerebral haemorrhage) with concurrent use of ginkgo and serotonergic antidepressants. Mechanism: additive platelet function inhibition plus possible interaction via monoamine oxidase inhibition. Combination only after careful benefit-risk assessment under medical supervision.
CYP3A4- und CYP2C9-Substrate (Ciclosporin, Tacrolimus, Statine, Calciumblocker, Midazolam u. a.)
In vitro and clinical studies suggest moderate CYP3A4 induction and possible CYP2C9 inhibition by ginkgo extract. Consequence: accelerated metabolism of ciclosporin (rejection risk in transplant patients) and other CYP3A4 substrates; altered exposure of CYP2C9 substrates (e.g. warfarin — clinically dual interaction risk). Assess clinical relevance case by case.
Insulin und orale Antidiabetika (Sulfonylharnstoffe, Glitazone, GLP-1-Analoga)
Animal and some clinical data suggest effects on insulin secretion and sensitivity by ginkgo extracts. Risk of hypoglycaemia or attenuation of effect possible. Monitor blood glucose regularly when combined.
Contraindications
- Hypersensitivity to Ginkgo biloba or other Ginkgoaceae; known allergy to urushiol-containing plants (poison ivy, mango skin oil) — increased risk of cross-reaction with ginkgolic acids.
- Pregnancy and lactation — strictly avoid therapeutic doses of ginkgo leaf extracts; seeds to be completely avoided during pregnancy and lactation.
- Children and adolescents under 18 years (leaf extracts); children of any age for ginkgo seeds.
- Existing coagulation disorders (haemophilia, thrombocytopaenia) or ongoing anticoagulant or antiplatelet therapy without explicit medical approval.
- Epilepsy or other seizure disorders — ginkgo seeds strictly contraindicated; leaf extracts only after careful medical assessment.
- Planned surgery or invasive procedures: discontinue ginkgo preparations at least 2 weeks before the procedure (bleeding risk).
Harvested part: leaves (autumn)
- Time of day:
- Collect on a dry sunny morning (9–12) once morning dew has evaporated. For medicinal purposes: green to slightly yellowing leaves in late summer to early autumn (August–September) — flavonol and terpene lactone content is highest then, before autumn yellowing sets in. Leaves should be unblemished and free from insect damage, mould and disease spots.
- Drying:
- Spread leaves carefully in single layers or thin layers on drying screens or cloths. Dry in shade, well ventilated, at max. 40 °C (cold air dryer setting or dehydrator low programme). Fully dried (approx. 1–2 weeks natural drying) and crumble dry. Ginkgolic acid content in leaves is relatively low, but wearing gloves when handling large amounts of fresh leaves is advisable, especially for allergy-prone individuals. After drying: pack tightly into glass or tin containers, store away from light.
- Storage:
- 12 months — Airtight, protected from light, cool and dry. Use within one year — flavonol glycosides and terpene lactones decline with prolonged storage.
- Time of day:
- Collect seeds (fruits) in autumn after full ripeness (September–October), when they fall from the tree or detach easily from branches. Collect ONLY under female trees (only these bear fruit). Wear thick rubber gloves — the fleshy, yellowish-orange seed coat (sarcotesta) smells of butyric acid/cheese and contains high levels of ginkgolic acids (potent contact allergen).
- Drying:
- Remove the fleshy outer coat (sarcotesta) IMMEDIATELY — best done outdoors or with good ventilation (strong smell). Wash hard inner stones (inner seed shell), air-dry at room temperature (approx. 1–2 weeks). Store dried kernels for culinary use. IMPORTANT: use seeds only cooked. Kernels can be frozen (−18 °C) for up to 6 months. For pharmaceutical purposes: ginkgo seeds are not used (ginkgotoxin risk) — standardised extracts are made exclusively from leaves.
- Storage:
- 6 months — Store dried kernels cool and dry (up to 6 months); for longer shelf life, freeze (up to 6 months at −18 °C). Always dispose of seed coat (sarcotesta) immediately — do not mix with household waste, as it poses an allergy risk to others.

Quantitative lead compounds of the leaf. Standardised EGb 761 extract contains 22–27 % flavonol glycosides (relative to extract). Antioxidant (free-radical scavenging), inhibit lipid peroxidation, reduce capillary permeability, endothelium-protective. Main fraction: quercetin-3-rutinoside (rutin), quercetin-3-glucoside, and analogous kaempferol and isorhamnetin glycosides.

Ginkgo-specific diterpene lactones with unique cage terpene structure (t-butyl group). Ginkgolide B is the most potent antagonist of platelet-activating factor (PAF) of known natural origin — PAF antagonism explains part of the anti-allergic, bronchodilatory, and platelet-inhibitory effects. Standardised: EGb 761 contains 5–7 % total terpene lactones. Ginkgolides contribute to the increased bleeding risk.

Sesquiterpene lactone and significant component of the terpene lactone fraction in EGb 761 (~2.6 % of extract). Neuroprotective (protects mitochondrial membrane integrity, inhibits GABA-A receptor modulation), improves cerebral circulation. Bilobalide is absent outside Ginkgoales — an evolutionarily unique compound.

Vitamin B6 antimetabolite that inhibits glutamate decarboxylase and reduces GABA synthesis. Present in ginkgo seeds (nuts), barely measurable in leaf extracts. Cause of poisoning cases ('shijin-boke' in Japan): nausea, vomiting, seizures. Particularly dangerous in children (higher relative dose, lower seizure threshold). Pyridoxine (vitamin B6) is the antidote for poisoning.

Oligomeric and polymeric proanthocyanidins constitute the largest polyphenol fraction by weight in ginkgo leaves, but are partly removed during manufacture of standardised extracts. Antioxidant and vasoprotective properties; contribute to the astringent effect of leaf infusions.

Shikimic acid is an intermediate in aromatic biosynthesis and present in notably high concentrations in green ginkgo leaves. Mildly antioxidant, likely of minor significance for the therapeutic effect of standardised extracts.
